APPLICATION INSTRUCTIONS:
- Complete the application
- Complete and sign the Personal Financial Statement on page 2 for each owner or principal of the business.
- Provide signed copies of the last business financial statements and Federal tax returns.
- Provide a copy of your most recent business deposit account bank statement.
- Provide a copy of your Articles of Incorporation, Partnership Agreement or Assumed Name papers.
- Provide proof of Tax ID Number.
- Return the required information to your nearest ELGA branch office and we will promptly process your Business VISA application
